I think my all time favorite (or least favorite, as it may be) was Tass Times in Tonetown where if you didn't get the book at the beginning of the game you were screwed when you got to the end: there was no way to go back and get the book and you had to have it to finish the game.

Ultima 7 had a bug when it first came out - when you slept there was a random chance of losing something in your backpack. If it was the key to finish the game you couldn't finish it.

What I do miss and would love to see back is something like the Laura Bow games - where you can finish it but there are so many 'extra' scenes if you are in the right place at the right time, that by playing it again you can see more character development.

There were simple things from hiding behind a curtain at the right time of night to overhear two characters discussing something, to the phone book which held the code to the safe among a list of 8 names. I can't remember the extra bit, but another name was a hint to another part of the evening if you worked it out and were in the right place at the right time.

Just general replay ability is what I loved. You play a good game and you want to play it again when you find out there's more to see. Just like Tex really.
